Intelicare Select Programs are for the consumer who does not qualify for a major medical and who is looking for well-rounded benefits at a lower cost.

Select 500 As a member of Consumer Health Choice Association (CHCA) and the Intelicare Select 500 program, you will have access to all of the Insurance Program Benefits, Patriot Health Florida, Inc.* Program Features, CHCA Lifestyle Membership Features and CHCA's FREE Membership Features.

Select 1000 As a member of Consumer Health Choice Association (CHCA) and the Intelicare Select 1000 program, you will have access to all of the Insurance Program Benefits, Patriot Health Florida, Inc.* Program Features* CHCA Lifestyle Membership Features and CHCA's FREE Membership Features.

Premium 500 As a member of Consumer Health Choice Association (CHCA) and the Intelicare Premium 500 program, you will have access to all of the Insurance Program Benefits (with the exception of the Mental Health benefit) Patriot Health Florida, Inc.* Program Features, CHCA Lifestyle Membership Features and CHCA's FREE Membership Features.
